Most conversations about male hormonal health focus on testosterone alone — whether it's high enough, how it compares to age-matched norms, whether symptoms warrant treatment. But testosterone doesn't exist in isolation. In men, a significant fraction of circulating testosterone is converted to estradiol by the enzyme aromatase, found in fat tissue, bone, liver, brain, and other tissues. The resulting estradiol plays essential roles in male health — protecting bones, supporting libido, regulating mood, and maintaining cardiovascular function. The balance between testosterone and estradiol, expressed as the T:E2 ratio, is increasingly recognized as more clinically meaningful than either hormone in isolation.
How the T:E2 ratio is calculated
The standard T:E2 ratio is calculated by dividing total testosterone in ng/dL by estradiol in pg/mL. This produces a unitless ratio that reflects the relative balance of the two hormones. The reason these specific units are used — rather than both converted to the same system — is convention: US lab reports almost universally express testosterone in ng/dL and estradiol in pg/mL, making this the most practical calculation for men working with their own results.
The formula is simply: T:E2 = Total Testosterone (ng/dL) ÷ Estradiol (pg/mL)
Research, including a review published in the World Journal of Men's Health, identifies an optimal range of 10 to 30 for this ratio in adult men. Values below 10 suggest estradiol is elevated relative to testosterone. Values above 30 suggest estradiol may be insufficient relative to testosterone — a less commonly discussed but clinically important imbalance.

What a low T:E2 ratio means (estradiol dominant)
A T:E2 ratio below 10 means estradiol is high relative to testosterone. This pattern — sometimes called estrogen dominance — is most commonly seen in men who are overweight or obese (since adipose tissue is the primary site of aromatase activity), men on testosterone replacement therapy without aromatase management, older men whose testosterone has declined while aromatase activity has increased, and men with liver disease or heavy alcohol use.
Symptoms associated with a low T:E2 ratio or elevated estradiol include gynecomastia (breast tissue development), water retention and puffiness, reduced libido, erectile dysfunction, mood changes including anxiety and emotional sensitivity, and fatigue. Importantly, these symptoms can occur even when estradiol is within the statistical reference range — if testosterone is simultaneously low, the relative imbalance is the issue.
What a high T:E2 ratio means (estradiol insufficient)
A T:E2 ratio above 30 means estradiol is low relative to testosterone. This is less commonly discussed but clinically significant. Estradiol is essential for male bone density — research has established that estradiol, not testosterone, is the dominant sex steroid protecting bone mass in men. Men with chronically low estradiol lose bone at a rate comparable to postmenopausal women. Other consequences of low estradiol in men include joint pain and stiffness, reduced libido, cognitive changes, mood disturbances, and cardiovascular risk.
Low estradiol in men most commonly results from low testosterone (since estradiol is produced by converting testosterone), excessive use of aromatase inhibitors during TRT, very low body fat, or congenital aromatase deficiency. Men on TRT who are also taking aromatase inhibitors to manage estradiol should be particularly vigilant about not over-suppressing E2.
The assay matters: Standard estradiol immunoassays were developed for women, where estradiol can reach 200–400 pg/mL during the menstrual cycle. At male concentrations — typically 10 to 40 pg/mL — standard assays are imprecise and frequently inaccurate. For reliable results, request the sensitive or ultrasensitive estradiol assay (LabCorp: "Estradiol, Sensitive"; Quest: "Estradiol, Ultrasensitive LC/MS"). This distinction can meaningfully change interpretation, particularly at the lower end of the male range.
What drives aromatization — and how to influence it
Aromatase activity — and therefore the rate at which testosterone converts to estradiol — is not fixed. The primary driver of aromatization in men is body fat, particularly visceral and subcutaneous abdominal fat. Men with higher body fat percentages produce more estradiol from the same amount of testosterone, shifting their T:E2 ratio downward. This is one of the most significant mechanisms by which obesity suppresses the testosterone-to-estradiol balance in men.
Other factors that increase aromatization include aging, alcohol consumption, zinc deficiency, and certain medications. Factors that reduce aromatization include weight loss, resistance training, reducing alcohol intake, and correcting zinc deficiency. Pharmaceutical aromatase inhibitors (anastrozole, exemestane) are effective but require careful medical management to avoid over-suppressing estradiol to the point of causing bone and joint problems.
T:E2 ratio in men on testosterone replacement therapy
The T:E2 ratio is particularly important for men on TRT, because exogenous testosterone significantly increases the substrate available for aromatization. As testosterone levels rise on TRT, estradiol typically rises proportionally — sometimes excessively in men with high aromatase activity, particularly those who are overweight. Monitoring the T:E2 ratio alongside absolute estradiol levels allows TRT prescribers to titrate therapy and, where necessary, adjust aromatase inhibitor dosing to maintain the hormonal balance within the optimal range without suppressing estradiol too aggressively.
